**Job Title: Medical Social Worker**

**Employment Type**: Full-time

**Position Overview**:
Key Responsibilities:
**1. Patient Assessment and Counseling**:

- Conduct thorough psychosocial assessments of patients and their families to identify needs, strengths, and support systems.
- Provide counseling and emotional support to patients and families dealing with illness, hospitalization, and post-discharge planning.

**2. Care Coordination**:

- Develop and implement individualized care plans in collaboration with the healthcare team.
- Coordinate with medical staff, nurses, therapists, and other healthcare providers to ensure comprehensive patient care.
- Facilitate communication between patients, families, and healthcare providers.

**3. Resource Linkage**:

- Identify and connect patients and families with community resources, financial assistance programs, and support services.

**4. Advocacy**:

- Advocate for patients’ rights and access to necessary services and treatments.
- Educate patients and families about their rights and options within the healthcare system.

**5. Discharge Planning**:

- Develop and implement discharge plans that address the medical, emotional, and social needs of patients.
- Arrange for post-discharge services such as home health care, rehabilitation, and follow-up appointments.

**7. Documentation and Compliance**:

- Maintain accurate and timely documentation of all patient interactions, assessments, care plans, and services provided.
- Ensure compliance with all relevant regulations and organizational policies.

**Qualifications**:

- Master’s Degree in Social Work (MSW) from an accredited institution.
- Previous experience in a healthcare setting, preferably in a hospital, clinic, or similar environment.
- Strong knowledge of medical terminology, healthcare systems, and community resources.
- Excellent communication, interpersonal, and organizational skills.
- Ability to work collaboratively as part of a multidisciplinary team.
- Empathy, compassion, and a commitment to patient advocacy.

**Job Types**: Full-time, Permanent

Pay: ₹15,000.00 - ₹18,000.00 per month
