**Key Accountabilities/ Responsibilities**
- The most important end-results for which an incumbent would be held responsible at the end of the year.
- Explain the features of the Relex Medication Therapy Management Program to prospective or existing customers/patients
- Scrutinize and digitize patient prescriptions and ensure thorough analysis and vetting before medication is dispensed
- Patient counselling and guidance on correct use of medication for achieving desired/targeted outcomes
- Closely monitoring patient progress through regular review patient outcomes and data, follow up on lab tests and their results. Intervention with patient and/or patient’s doctors where necessary
- Communicate customer feedback and customer experience on products/services and/or modifications to existing products/services to Operations, Marketing and Quality teams.
- Must be able to work both with external and internal customers.
- Comfortable in new environments and adverse conditions. Must be able to work and interact with diverse groups of people.
- Provide information and explanation of diagnoses, options, outcome and risk and escalate where required
- Data updation in all tracking platforms on a daily basis
- Prescription digitization analysis and scrutiny and preparation on reference documents on their medication for patients registered on Relex Medication Therapy Management Program
- Counselling of patient based on documents prepared and recording initial tracking information on outcomes. Placement of initial medicine requirements or fresh requirements on prescription change
- Review of patient progress and initiating counselling notes for follow up on forthcoming visits by Pharmacist cum HCA. Intervention with patient or their doctor on matters deserving escalation.

**Salary**: ₹20,000.00 - ₹25,000.00 per month
