**Roles & Responsibilities**:

- Critical Time Intervention - Follow the protocol
- Facilitate referral process with other organizations/families
- Complete documentation for each referral - Consent Form, Nominated Representative, Advance Directives
- Provide case management for new clients
- Obtaining Police Intimations, Reception Orders & Memos for new admissions.
- Renewal of Reception Orders on a monthly basis
- Intimate the police about missing clients within 12 hours.
- Monitoring Food Serving during Dinner & Tea serving every day.
- Clients Cupboard checking to be done weekly three days.
- Follow-up on the new clients progress
- Documentation of New Client’s Files
- ECRC Statistics register
- Maintain Asset register.
- Create Event Calendar - Quarterly
- Facilitate Aadhaar card and disability card.
- Organize All Residents outing once in a quarter.
- Individual sessions
- Psycho-social Interventions
- Documentation
