**Key Responsibilities**:

- **Clinical Pharmaceutical Services**:

- Provide pharmaceutical care through comprehensive medication therapy management, reviewing patient profiles, and identifying potential medication-related issues.
- Collaborate with healthcare providers to optimize patient medication regimens, making recommendations based on the latest evidence and clinical guidelines.
- Participate in multidisciplinary rounds, offering expert advice on drug therapy, dosages, drug interactions, and side effects for hospital patients.
- **Patient Safety and Medication Management**:

- Oversee the preparation and dispensation of medications for inpatients and outpatients, ensuring the correct medication, dosage, and administration method.
- Monitor drug therapy for therapeutic effectiveness, adverse effects, and compliance.
- Review and assess lab results and medical histories to recommend alternative drug therapies, if necessary.
- **Education and Patient Counseling**:

- Provide patients and caregivers with in-depth counseling on the safe use of medications, including instructions for self-administration, side effect management, and any required lifestyle adjustments.
- Educate healthcare staff on pharmacological updates, medication management, and safe practices.
- **Inventory and Supply Chain Management**:

- Manage hospital pharmacy inventory to ensure the availability of medications, ensuring proper stock rotation, timely orders, and management of expired drugs.
- Work closely with the procurement team to ensure that medication supply levels meet hospital needs and avoid shortages.
- **Regulatory Compliance and Documentation**:

- Ensure strict adherence to all regulatory and hospital policies regarding medication dispensing, storage, and record-keeping.
- Maintain patient records in accordance with HIPAA and other healthcare privacy regulations.
- Ensure compliance with hospital accreditation standards, including medication safety, quality control, and infection prevention.
- **Leadership and Training**:

- Supervise, mentor, and train junior pharmacists and pharmacy technicians, providing guidance on best practices and professional development.
- Participate in hospital committees and quality improvement initiatives focused on medication safety and patient care.

**Qualifications**:

- **Education**:

- Doctor of Pharmacy (Pharm.D.) degree from an accredited institution.
- **Licensure**:

- Valid Pharmacist license in [state/country].
- **Experience**:

- A minimum of 5 years of experience working as a pharmacist in a corporate hospital setting, with expertise in clinical pharmacy practices, medication management, and interdisciplinary collaboration.
- **Skills**:

- In-depth knowledge of pharmacology, drug therapy management, and evidence-based clinical practices.
- Strong communication skills to interact effectively with physicians, nurses, and patients.
- Ability to manage complex medication regimens and provide clear explanations to patients and healthcare teams.
- Proficiency in hospital pharmacy software and electronic medical record (EMR) systems.
