**Key Responsibilities**:

- **Patient Support: Provide emotional and social support to patients and their families, helping them cope with the challenges associated with their healthcare journey.**

- **Resource Coordination: Identify and connect patients with relevant community resources, including financial, housing, and transportation assistance, as needed.**
***:

- **Assessment: Conduct psychosocial assessments of patients to determine their specific needs and develop care plans accordingly.**
***:

- **Collaboration: Work closely with healthcare providers, including doctors, nurses, and case managers, to ensure comprehensive care for patients.**
***:

- **Documentation: Maintain accurate and confidential patient records, documenting assessments, interventions, and care plans.**
***:

- **Crisis Intervention: Assist in resolving crises or emergencies, providing immediate support to patients and families in distress.**
***

**Qualifications***:

- **Bachelor’s degree in Social Work or a related field.**
***:

- **Relevant experience (2-3 yrs) as a Medical Social Worker in a healthcare setting.**

- **Excellent English communication skills, both written and verbal.**
***:

- **Strong interpersonal and listening skills.**

- **Empathetic and compassionate attitude.**

- **Ability to work effectively in a team and independently.**
