Coimbatore, India Decipher Health Records LLP Full time

We are looking for Freshers and MT (Medical Transcriptionists). Training will be provided for Scribing. Training duration will be TWO months for MTs and FOUR months for Freshers. People who are looking for a change in their domain can also apply.

**JOB DESCRIPTION**:

- Scribes are assigned to work with one physician per shift.
- Scribes must accompany the physician upon patient interview and examination.
- Scribes document the physician-dictated patient history, including history of present illness, review of systems, past medical and surgical history, family and social histories, medications and allergies, physical examination findings and procedures as performed by the physician, results of laboratory and radiographic studies as dictated by the physician, correct time of patient care related activities, including physician to physician communication, family communication and re-examination of the patient.
- When the physician concludes the patient’s encounter, the physician will review all documentation completed by the Scribe, make any necessary amendments, and sign the chart. The physician is ultimately responsible for documentation of the patient’s encounter.
- The physician and the Scribe will make “chart rounds” to review patient status, delays, and other care-related issues.
- All orders on patient care must be communicated only by the physician and not the Scribe.
- Scribes should not participate in any patient care and should only refer all requests related to patient care to the responsible physician or nursing staff, including, but not limited to, transporting specimens, answering phones, assisting patients, calling physicians, etc.

**JOB REQUIREMENTS**:

- **Good verbal and written communication is a must.**:

- Typing speed with a minimum of 60 WPM without looking at the keyboard and with proper fingering.
- **Willingness to work fixed night shifts.**:

- Immediate joiners are preferred.
- **Willingness to relocate near the locality**

**WORK LOCATION: R.S. PURAM, COIMBATORE, TN**

**WORK TYPE: FULL-TIME & WORK FROM OFFICE**

**Salary**: ₹25,000.00 - ₹35,000.00 per month
