Description
- Conduct comprehensive assessments of patients' health status, including physical, emotional, and psychosocial aspects, to develop individualized care plans and goals.
- Provide skilled nursing care to patients in their homes, including but not limited to wound care, medication management, IV therapy, catheter care, and palliative care as needed.
- Monitor patients' condition and response to treatment, and collaborate with physicians and other healthcare professionals to adjust care plans and interventions as necessary.
- Educate patients and their families on disease management, medication administration, nutrition, safety precautions, and other aspects of self-care to promote optimal health and independence.
- Administer medications and treatments as prescribed by healthcare providers, ensuring accuracy, safety, and adherence to established protocols and regulations.
- Coordinate with interdisciplinary team members, including therapists, social workers, and home health aides, to ensure coordinated and holistic patient care.
- Document all patient care activities, assessments, and interventions accurately and timely in electronic health records (EHR) or other designated systems, in compliance with regulatory standards and agency policies.
- Provide emotional support and counseling to patients and their families, addressing concerns, providing resources, and facilitating access to community services as needed.

Pay: ₹18,000.00 - ₹25,000.00 per month
