**Roles and Responsibilities**:
- Part of the dedicated team to assist **during onboarding and prescription updates**:
- **Scan through discharge summary and guidelines by the Cardiologist to fill the Customised Health Package**:
- Ensuring 100% accurate entry of inputs for the following with clinical context using the discharge summary
- Diagnosis
- Clinical summary
- Devices needed
- Diagnostics needed
- Frequency of monitoring
- Target and threshold values
- Medication regime
- Refer to the guidelines by the cardiologist for lifestyle modifications and **accordingly choose the modules needed for the patient**:
- Incorporating changes to the Customised Health Package after first review by the Cardiologist
- Ensuring final validation/approval of the Customised Health Package by the Cardiologist via the company POC
- To **review and approve any digitization** of uploaded prescriptions or prescription changes to ensure 100% accuracy in each entry
**Skills and Requirements**:
- **Degree**:
- Bachelor of Medicine and Bachelor of Surgery
- Fellowship in Clinical Cardiology is a plus
- **Experience**: One plus year experience in Government/Private hospital
- **Language**: English, Hindi or Marathi
- **Skills**:
- Clinical understanding of post ACS cases to ensure accurate entries
- Ability to work in cross functional settings and coordinate with variety of stakeholders like doctors and operations team
- Tech savvy and can work with computers, phones, and tablets in a remote setting
- Clear communication skills to interact with company POC and Cardiologist to ensure timely changes are incorporated
- Strong computer skills with knowledge of Microsoft Programs such Word, PowerPoint, Excel etc
- 6 days working, 5 days from office and Sat is WFH
