**Key Responsibilities**
- Review and analyze E&M documentation for IP & OP records to assign accurate diagnosis and procedure codes.
- Ensure coding compliance with industry standards, client-specific protocols, and regulatory guidelines.
- Utilize ICD-10-CM, CPT, and HCPCS Level II classification systems for accurate code assignment.
- Maintain minimum productivity and accuracy benchmarks as defined by the organization.
- Work closely with auditors, QA, and training teams to enhance coding quality.
- Escalate documentation discrepancies or unclear cases for physician clarification.
- Maintain up-to-date knowledge of coding and billing regulations.
- Support team members in resolving coding-related issues.

**Required Skills & Qualifications**
- **1 to 3 years** of hands-on experience in E&M coding (IP & OP).
- Strong knowledge of **ICD-10-CM**, **CPT**, and **HCPCS Level II**.
- Familiarity with **Medicare, Medicaid, and commercial insurance** guidelines.
- Certification preferred**:CPC, CCS, COC, or equivalent (AAPC/AHIMA)**.
- Working knowledge of medical terminology, anatomy, and healthcare documentation.
- Strong communication and analytical skills.
- Proficiency in using EMR/EHR and coding tools.
