Working with the Early Intervention Centre Project Head and the Director, Education_, _the **Inclusion Co-ordinator (IC) **is responsible for facilitating transition of children from our centres into mainstream schools. This role has two components. The IC will create a strong partnership with relevant **staff and parents** of students transitioning into a mainstream setting by ensuring that the guidelines of the Inclusion Toolkit are followed and its resources used to facilitate a smooth transition. In addition, the IC will monitor and track the progress of students involved in the Inclusion project from the previous academic year(s) and develop a long term plan to work with all children across centres who are ready for mainstream settings. Follow up on those who already transferred is critical.
