Portea Medical is a leading healthcare provider specializing in home healthcare services. We are currently seeking a dedicated and experienced Staff Nurse to join our dynamic team and provide high-quality nursing care to patients in a home-based setting.
As a Staff Nurse, you will play a pivotal role in ensuring patients' well-being and recovery, assisting with their daily medical needs, and delivering compassionate, patient-centered care in collaboration with physicians and other healthcare professionals.
**Key Responsibilities**:
Tracheotomy Care: Provide expert care for patients with tracheostomy tubes, including cleaning, suctioning, and monitoring for complications. Ensure proper airway management and assist with tracheotomy tube replacement as per clinical guidelines.
Colostomy Care: Manage colostomy patients by teaching proper stoma care, monitoring for signs of infection, and ensuring the comfort and safety of patients. Provide emotional and psychological support as needed.
Central Line Care: Perform sterile dressing changes, monitor for signs of infection, and ensure the maintenance of central venous access devices (CVCs). Assist in the safe administration of intravenous medications and fluids.
Feeding and Nutrition Support: Administer enteral feeding (via nasogastric or gastrostomy tubes), monitor patient tolerance, and report any concerns related to feeding tubes, aspiration, or gastrointestinal issues.
CPAP & BIPAP Monitoring: Set up and manage continuous positive airway pressure (CPAP) and bilevel positive airway pressure (BiPAP) devices for patients with respiratory issues. Monitor patient adherence and comfort, and adjust settings as prescribed by the healthcare provider.
Vital Monitoring: Regularly assess vital signs, including heart rate, blood pressure, oxygen saturation, temperature, and respiratory rate. Report any abnormal findings to the supervising physician or medical team promptly.
Patient Education: Educate patients and their families on managing medical conditions, proper use of equipment,and ensuring comfort and safety at home. Provide emotional support and ensure patient and family understanding of care plans.
Emergency Response: Be prepared to respond to any changes in the patients condition, including administering emergency procedures as per protocol.
Documentation & Reporting: Maintain accurate and up-to-date patient records, including nursing notes, care plans, and patient progress reports. Ensure timely reporting of any changes in patient health to the medical team.
