**Job Title: Hospital Pharmacist**
Job Summary:
A Hospital Pharmacist is responsible for managing and dispensing medications within a hospital setting. They work closely with healthcare professionals to ensure patients receive the right medications safely and effectively.
Key Responsibilities:
1. Medication Dispensing: Accurately dispense medications to patients, ensuring the right medication, dose, and quantity.
2. Medication Management: Manage medication inventory, including ordering, storage, and rotation of stock.
3. Patient Counseling: Provide patients with information about their medications, including dosage, side effects, and interactions.
4. Collaboration with Healthcare Teams: Work with doctors, nurses, and other healthcare professionals to optimize patient care and medication therapy.
5. Medication Safety: Monitor and report medication errors, adverse reactions, and near misses to ensure patient safety.
6. Pharmacovigilance: Identify and report adverse drug reactions to regulatory authorities.
7. Medication Policy Development: Contribute to the development and implementation of medication policies and procedures.
Requirements and Qualifications:
1. Pharmacy Degree: A degree in pharmacy (B.Pharm or Pharm.D.) from a recognized institution.
2. Registration: Registration with the relevant pharmacy council or board.
3. Experience: Previous experience in a hospital pharmacy setting is preferred.
4. Strong Knowledge: Strong knowledge of pharmacology, therapeutics, and medication management.
5. Attention to Detail: Attention to detail and ability to work accurately in a fast-paced environment.
