**Key Responsibilities**:
- Assess and monitor physical, emotional, and spiritual needs of patients receiving palliative care.
- Develop and implement individualized care plans in collaboration with patients, families, and the healthcare team.
- Administer medications and treatments for symptom control and pain relief.
- Educate patients and families on disease progression, care options, and end-of-life planning.
- Provide psychosocial support and counseling as needed.
- Coordinate care with physicians, social workers, chaplains, and other healthcare professionals.
- Advocate for the patient's wishes and ensure dignity and comfort are maintained.
- Maintain accurate and timely documentation of care provided.
- Participate in family meetings, care planning conferences, and case reviews.
- Support bereavement care for families after patient death, when applicable.
