**Salary**: ₹15,000 - ₹20,000 per month

24x7 Care Foundation is dedicated to improving the lives of individuals with disabilities and elderly individuals in our community. We are currently seeking compassionate and dedicated individuals to join our team as Social Workers specializing in Disability and Elderly Services. In this role, you will provide vital support, advocacy, and resources to enhance the quality of life for both disabled and elderly individuals.

**Responsibilities**:

- Conduct comprehensive assessments to identify the unique needs and strengths of individuals with disabilities and elderly individuals.
- Develop and implement individualized care plans tailored to meet the specific needs of each client.
- Provide emotional support, counseling, and guidance to individuals and families facing challenges related to disability and aging.
- Assist individuals with disabilities and elderly individuals in accessing community resources, including healthcare, housing, transportation, and social services.
- Advocate for the rights and interests of individuals with disabilities and elderly individuals within the community and healthcare systems.
- Collaborate with interdisciplinary teams, including healthcare professionals, caregivers, and social service agencies, to coordinate holistic care and support for clients.
- Maintain accurate and confidential client records and documentation.
- Participate in ongoing training and professional development opportunities to enhance skills and knowledge in the field of disability and elderly services.

**Requirements**:

- Bachelor’s degree in Social Work or related field (RCI registration is mandatory).
- Passion for working with individuals with disabilities and elderly individuals.
- Excellent communication and interpersonal skills.
- Strong organizational and problem-solving abilities.
- Ability to work independently and as part of a team.
- Compassionate, empathetic, and nonjudgmental approach.
