Assess the pre-existing health conditions and evaluate the health issues of the patients to provide effective solutions and setting goals to improve their health status.
- Counsel and advise patients or healthy people to improve the overall health condition
- Suggest exercises, diet charts, meditation, fitness regimes or mental stress reducing techniques to clients
- Coach and motivate patients to follow their individualised care plans and pathways, supporting them to make proactive healthy choices
- Identify the need for behavioral change to improve health status, reduce health risks and improve quality of life
- Work with other care providers to identify opportunities for improvements in patients’ health and devise appropriate care plans.
- Work closely with the customer health and wellness team in meeting the organization's goals.
- Ensure continuous patient engagement and support them throughout their patient journey.
- Promote quality outcomes by managing care needs through education and health advocacy.
- Taking charge of home based care of assigned patients and ensuring tele-consultations, nursing, physiotherapy, diagnostics, specialty referrals and other services have been executed and results received and acted upon as needed.

**Salary**: ₹30,000.00 - ₹70,000.00 per month
