**Key Responsibilities**

The provide services for movement of the patients’ files inside or outside the organization according to requisitions.

To file the patient’s documents whichever comes after the discharge of patients.

The arrangements of files according to IP numbers for the systematic records management of the patient.

The write Registration and IP numbers of the systematic records management of the patients.

To provide files or photocopy of patients records whenever required by organization’s people like Clinicians, employee etc. or people from outside the organization like patients’ relatives, legal body etc.

To assist the maintenance of proper pest control records of department.

To do other miscellaneous works like to make proper arrangement in order to keep Medical Records Department clean, to collect various documents from the various departments for smooth working of Medical records Department.

Retrieval of records to physicians and concerned personnel for reference/review.

Review of records periodically for proper functioning of Department.

To receive patient’s medical records files after their discharge (IPD Cases / OPD Cases).

Checking the Medical records files as per checklist for completion and getting the incomplete records completed without any delay.

Entry of clinical parameters in the system for developing quality indicator.

Solving queries of patients and relatives during and after discharge of patients related to medical records.

Retrieval of records to physicians and concerned personnel for reference/review.

To provide medical record documents to discharged patients/TPA/Insurance patient from file as per general purpose form.
Any other work as assigned by Sr.Executive (MRD) Clinical Care/Head Clinical Services related to MRD

**Job Types**: Full-time, Regular / Permanent

**Salary**: Up to ₹20,000.00 per month

- Rotational shift

Ability to commute/relocate:

- Ahmedabad, Ahmedabad - 382481, Gujarat: Reliably commute or planning to relocate before starting work (required)

**Education**:

- Bachelor's (preferred)

**Experience**:
