**Description**
- Conduct comprehensive nursing assessments of patients, including physical assessments, health histories, and psychosocial evaluations, to develop individualized care plans.
- Implement nursing interventions, including medication administration, wound care, IV therapy, catheterization, and patient education, according to established protocols and standards of practice.
- Monitor patient vital signs, symptoms, and laboratory values, and assess for changes in patient condition, responding promptly to signs of deterioration or complications.
- Collaborate with physicians, nurse practitioners, and other healthcare professionals to coordinate patient care, communicate patient status updates, and ensure continuity of care across shifts and disciplines.
- Provide patient education and counseling on health conditions, treatment plans, medications, self-care strategies, and preventive measures to promote patient understanding and compliance.
- Document all patient assessments, interventions, and responses to treatment accurately and comprehensively in the electronic medical record (EMR), ensuring adherence to regulatory requirements and standards of documentation.
- Assist with patient admissions, discharges, and transfers, including completing admission assessments, discharge planning, and patient handoffs to ensure safe transitions of care.
- Maintain a safe and therapeutic environment for patients, families, and healthcare team members by adhering to infection control protocols, safety guidelines, and regulatory standards.
