Screen new patients to assess their medical, financial, emotional, social and environmental needs
- Identify high-risk patients and determine if they have critical health conditions, chronic illnesses or terminal illnesses
- Determine if patients require expensive treatments or if they may not comply with treatment plans
- Find out if it is necessary to address any specific health needs or issues
- Meet with patients and their families to discuss health conditions, treatments, options and available resources
- Coordinate care plans and any services the patients may require with the medical staff
- Advocate for the patients with nurses, doctors, physical therapists and administrative staff
- Provide advice and counselling to patients and their families about living with injuries or illnesses
- Perform case management tasks for patients, check their documentation and ensure the information is accurate
- Assist patients in arranging finances for their medical treatments
- Help patients to obtain medication for their health conditions
- Accompany discharged patients from healthcare facilities on their return home
- Assist patients with finding housing that suits their health requirements
- Find nurses to ensure that patients receive suitable care at home.
- Organising awareness camps in the field.

Pay: ₹7,000.00 - ₹8,000.00 per month
