Job Title CHF Counselor Reporting To Project Manager

Position Responsibility summary:

- HF Counselor to attend identified Doctor Clinic in OPD for Patient counseling.
- HF Counselor will conduct a risk assessment test with the KRIBADO machine
- HCAH is to maintain the patient’s data in the system
- Doctors consent with whom clinics are being conducted
- Any change in the schedule of Clinic OPD to be conducted post approval from PL/PM
- Based on HCAH CRM functionality HF Counselor will get the notification of the camp schedule
- On the scheduled date of OPD, HF Counselor will start the visit in the CRM which will send an automatic notification to the HCAH project head
- The ending of the OPD will be notified automatically to the project head
- The Project manager will send a summary report for all the visits with details on the next day to

CIPLA
- HF Counselor will brief the patient about App services
- HF Counselor will guide the patients with Digital counseling, Diet and Lifestyle counseling,

Physiotherapy and Yoga sessions, etc.

Responsibility
- Attend Doctors OPD at Clinic/Hospital (Twice a day Approx 40 - 45 OPD / Month)
- Interaction with Patients for risk assessment tests by using the KRIBADO machine
- Coordinating with the internal team, Project Lead/ RM, and Doctors on daily basis.
- Submission of Admin work like Daily Reporting, Sending Consent Forms (via mail daily, Courier monthly), and Activity Sheets to Head Office on time.

Project KPI
- Ensure a minimum of 10-12 Clinic OPD are conducted every week
- Feedback from doctors & patients
- Continuous Increase in no. of patients attending education in OPD
- Assist patient with the HCAH App and its services
- Expected up to 8 hours per day of working time (Anytime between 9 am to 9 pm)

Qualification
- Should be a Certified Dietician with 1-2 years of experience
- Experience in Cardiac Clinic would be ideal
