Specific roles and responsibilities
To ensure that trained caregivers are successfully placed in relevant caregiving roles and receive ongoing
mentoring, counselling, and support to foster their professional development and enhance patient outcomes.
1. Job Placement and Post-Course Support
- Facilitate the placement of trained caregivers in caregiving roles within healthcare facilities, old-age
homes, and in-home care settings.
- Develop and maintain partnerships with organizations and agencies to secure placement opportunities
for trainees.
- Communicate regularly with partner organizations to ensure smooth placement processes and address
any initial challenges faced by caregivers.
2. Mentoring and Monitoring Visits
- Conduct regular on-site mentoring visits to monitor the progress of caregivers in their placements.
- Provide hands-on guidance and practical advice to caregivers, helping them to adapt to their roles and
overcome any role-specific challenges.
- Document mentoring sessions, track the performance and progress of caregivers, and provide feedback
to improve their caregiving skills.
3. Ongoing Handholding and Counselling
- Offer one-on-one counselling sessions to caregivers to address work-related challenges, stress
management, skill gaps, and other issues.
- Facilitate group sessions or peer-to-peer support networks to encourage knowledge sharing and mutual
support among caregivers.
- Provide additional resources or training opportunities for caregivers as needed to enhance their skills and
job satisfaction.
4. Feedback and Program Refinement
- Collect feedback from both caregivers and partner organizations to evaluate the program’s effectiveness
and identify areas for improvement.
- Analyze feedback and prepare reports on caregiver satisfaction, patient outcomes, and overall program
impact.
- Collaborate with the project team to refine the curriculum, improve placement processes, and enhance
post-placement support based on feedback.
5. Tracking Long-Term Impact
- Develop and maintain a tracking system to monitor the long-term career progression of caregivers and
assess their impact on patient care.
- Conduct follow-up evaluations at regular intervals to measure caregivers' professional growth and the
ongoing relevance of the training program.
- Prepare periodic reports on the long-term impact of the caregiving workforce, including data on
caregiver retention, career advancement, and patient outcomes.
Deliverables
- Monthly Reports: Detailed reports on placements, mentoring visits, counselling sessions, and feedback
from caregivers and partner organizations.
- Tracking Database: A comprehensive database tracking caregiver placements, career progression, and
feedback.
- Impact Assessment Reports: Periodic impact assessment reports evaluating the effectiveness of the training
program on both caregivers and patient outcomes.
- Program Refinement Proposals: Recommendations for improvements to the training curriculum, placement
strategies, and support mechanisms based on collected feedback.
Key Performance Indicators (KPIs)
- Successful Placements: Number of caregivers placed in roles within healthcare and elder care facilities.
- Mentoring and Support: Quality and frequency of mentoring visits, counseling sessions, and peer support
networks.
- Feedback and Improvement: Effectiveness in collecting feedback, proposing program improvements, and
implementing changes to enhance caregiver satisfaction.
- Long-Term Tracking: Accuracy and comprehensiveness of the tracking system for long-term caregiver
impact and career progression.
Essential Qualifications and Experience:
- Bachelor’s degree in Social Work, Healthcare Management, Human Services, or a related field. A Master’s
degree is an advantage.
- Minimum 2 years of experience in program coordination, social work, counselling, or a similar role within
the nonprofit or healthcare sector.
- Strong interpersonal skills, with the ability to mentor, motivate, and counsel individuals from diverse
backgrounds.
- Excellent organizational skills, with an ability to manage multiple tasks and priorities effectively.
- Proficiency in Microsoft Office Suite and experience with data management systems.
- Ability to conduct field visits and work independently, with a strong commitment to achieving program
objectives.
