**Job Summary**:
**Key Responsibilities**:

- Medication Management:

- Dispense prescribed medications accurately and ensure proper labeling and packaging.
- Review medication orders for appropriateness, including dosage, frequency, and potential drug interactions.
- Prepare and compound medications, including intravenous solutions, as needed.
- Patient Counseling:

- Provide counseling to patients regarding the safe and effective use of their medications.
- Educate patients on potential side effects, interactions, and proper storage of medications.
- Address patient inquiries and concerns related to their medications.
- Clinical Collaboration:

- Work closely with physicians, nurses, and other healthcare professionals to develop and implement individualized treatment plans.
- Participate in multidisciplinary team meetings to discuss patient care and optimize medication therapy.
- Review and monitor patient profiles to identify and resolve medication-related problems.
- Medication Safety:

- Ensure compliance with hospital policies, procedures, and regulatory requirements related to medication management.
- Monitor and report adverse drug reactions and medication errors in a timely manner.
- Maintain accurate and up-to-date medication records and documentation.
- Inventory Management:

- Manage medication inventory, including ordering, storing, and tracking medications.
- Conduct regular inventory audits and ensure proper storage conditions for all medications.
- Assist with the disposal of expired or unused medications according to regulatory guidelines.

**Education and Training**:

- Provide education and training to healthcare staff on new medications, treatment protocols, and best practices.
- Stay current with advances in pharmacotherapy and continuously update knowledge on new medications and treatment guidelines.

**Qualifications**:
Education: Bachelor of Pharmacy (B. Pharma) from an accredited institution.

Experience: Minimum of 1 year of experience as a pharmacist in a hospital or clinical setting.

Licensing: Valid pharmacy license as required by local regulatory authorities.

**Skills**:

- Strong knowledge of pharmacology, medication therapy management, and clinical pharmacy practices.
- Excellent communication and interpersonal skills, with the ability to effectively interact with patients and healthcare professionals.
- Detail-oriented with strong organizational and problem-solving skills.
- Ability to work independently and as part of a team in a fast-paced environment
