**Job Title: Diploma /Certificate in Community and Rehabilitation Therapist**

**Job Summary**:
The Certificate in Community and Rehabilitation Therapist (CCRT) provides comprehensive rehabilitation services to individuals with disabilities. The role involves assessing clients' needs, developing and implementing individualized care plans, and facilitating their physical, emotional, and social rehabilitation. The CCRT works closely with a multidisciplinary team to enhance children' quality of life and promote their independence.

**Key Responsibilities**

**Assessment and Planning**:

- Conduct thorough assessments of children's physical, emotional, and social needs.
- Develop personalized rehabilitation plans in collaboration with children's their families, and other healthcare professionals.
- Set realistic and achievable goals for children's rehabilitation.

**Therapeutic Interventions**:

- Implement a range of therapeutic interventions, including physical therapy, occupational therapy, and speech therapy.
- Provide education and training to clients and their families on managing disabilities.
- Monitor and evaluate children's progress, adjusting care plans as necessary.

**Coordination and Collaboration**:

- Work closely with other healthcare professionals, including doctors, nurses, social workers, and psychologists.
- Coordinate with community resources and support services to ensure comprehensive care.
- Participate in case conferences and multidisciplinary team meetings.

**Documentation and Reporting**:

- Maintain accurate and up-to-date records of clients' assessments, care plans, and progress.
- Prepare reports for internal use and external agencies as required.
- Ensure compliance with all relevant regulations and standards.

**Advocacy and Support**:

- Advocate for clients' rights and access to necessary services and resources.
- Provide emotional support and counseling to clients and their families.
- Promote awareness and understanding of disabilities within the community.

**Qualifications**:
**Education**:
Diploma/Certificate in Rehabilitation Therapy, Occupational Therapy, Physical Therapy, or a related field.

Certification and licensure as required by RCI regulations.

**Experience**:
Open for freshers as well experience working with individuals with disabilities.

Experience in a community care setting is preferred.

**Skills**:
Strong assessment and analytical skills.

Excellent interpersonal and communication skills.

Ability to work effectively in a multidisciplinary team.

Knowledge of disability-related services and resources.

Compassionate and patient-centered approach.

**Physical Requirements**:

- Ability to perform physical activities related to therapy, including lifting and supporting children's.
- Willingness to travel within the community to provide services.
