**Key Responsibilities**:

- Act as a liaison between patients, families, and healthcare teams to ensure seamless communication and effective treatment plans.
- Serve as a campaigner for patients, addressing their needs, concerns, and questions regarding their care, while ensuring they are treated with dignity and respect.
- Organize and schedule patient appointments, tests, procedures, and follow-ups, ensuring timely care delivery.
- Work with medical professionals to develop and monitor individualized care plans, ensuring they are followed and updated as necessary.
- Assist in discharge planning, ensuring patients understand post-care instructions, prescriptions, and follow-up appointments.
- Maintain accurate patient records and update databases with information on care, treatment, and patient progress.
- Collaborate with doctors, nurses, social workers, and other hospital staff to ensure comprehensive and coordinated care.
- Provide patients and their families with necessary information regarding their condition, treatment options, and healthcare resources.

**Qualifications**:

- Degree or Bachelor’s Degree (preferred): Healthcare Administration, Nursing, Social Work, Public Health & Medical Assisting.

**No of Requirement**:

- 2 Numbers

**Experience**:

- Experience - 0 to 10 Years Experiences

**Job Types**: Full-time, Permanent, Fresher

Pay: ₹10,000.00 - ₹15,000.00 per month
