**Key Responsibilities**:

- Assess, plan, implement, and evaluate individualized nursing care for home-bound patients.
- Administer medications, wound care, injections, and other nursing interventions as per physician's orders.
- Monitor vital signs, medical conditions, and recovery progress.
- Educate patients and families on disease management, medication adherence, and post-discharge care.
- Maintain accurate documentation and patient records according to hospital and legal standards.
- Coordinate with physicians, therapists, and social workers for comprehensive care planning.
- Ensure infection control practices are followed in the home environment.
- Provide emotional support to patients and caregivers, promoting independence and dignity.
- Respond to emergencies and escalate concerns promptly.
- Adhere to hospital policies, procedures, and regulatory standards.

**Qualifications**:

- **Education**: Diploma/BSc in Nursing from an accredited institution.
- **License**: Valid [State/National] Nursing License (e.g., RN).
- **Experience**: Minimum 1-2 years of clinical experience, preferably in home care, community health, or medical-surgical nursing.
- **Skills**:

- Strong clinical and assessment skills
- Excellent communication and interpersonal abilities
- Ability to work independently with mínimal supervision
- Time management and organizational skills
- Proficiency with electronic health records (EHR)

**Working Conditions**:

- Regular travel to patient homes within the service area.
- Flexible scheduling including evenings or weekends as needed.
- May involve exposure to various home environments and physical activities.

**Preferred Qualifications**:

- Certification in Home Health Nursing or Geriatric Care (optional but advantageous)
- Experience in palliative care or chronic disease management
- Fluency in multiple languages is a plus

Pay: From ₹32,000.00 per month
