**Responsibilities**:

- **Medication Management**:

- Review and process medication orders within our digital platforms, ensuring accuracy and adherence to clinical guidelines.
- Collaborate with healthcare providers to optimize medication therapy and patient outcomes through evidence-based practices.
- **Clinical Support**:

- Provide clinical expertise and guidance to interdisciplinary teams regarding medication-related issues.
- Conduct medication therapy management (MTM) consultations to optimize medication regimens and improve patient adherence.
- **Quality Assurance**:

- Ensure compliance with regulatory requirements and best practices in medication safety and management.
- Participate in quality improvement initiatives to enhance the medication-use process and patient care outcomes.
- **Patient Education**:

- Educate patients on the appropriate use of medications, potential side effects, and interactions.
- Support patient engagement efforts by providing clear and actionable medication information through digital channels.
- **Technology Integration**:

- Collaborate with our technology team to integrate pharmacy services into our digital health platforms.
- Utilize health information technology to streamline medication management processes and improve workflow efficiencies.
- **Continuous Learning**:

- Stay updated with the latest developments in pharmacy practice, health technology, and regulatory changes.
- Participate in continuing education and professional development activities to enhance skills and knowledge.

**Qualifications**:

- Licensed Pharmacist with Indian pharmacy license in good standing.
- Minimum of 1 years of experience in pharmacy practice, preferably in a clinical or digital health setting.
- Strong understanding of pharmacotherapy principles, drug information resources, and clinical guidelines.
- Excellent communication skills with the ability to interact effectively with healthcare providers and patients.
- Proficiency in using digital health platforms and electronic medical records (EMRs).
- Commitment to patient safety, quality care, and continuous improvement.

**Preferred Skills**:

- Board Certification in a pharmacy specialty (e.g., Ambulatory Care, Pharmacotherapy).
- Experience with medication therapy management (MTM) and medication reconciliation processes.
- Familiarity with health informatics and data analytics in healthcare.
