**Job Summary**
The **Hospital Pharmacist** is responsible for ensuring the **safe, effective, and economical use of medications** across inpatient and outpatient services. The role involves **dispensing medications, verifying prescriptions, preventing drug interactions, monitoring therapies**, and **counselling patients** to promote rational drug use and patient safety.
**Key ResponsibilitiesInpatient Services**
- Review physician prescriptions for accuracy, suitability, and potential interactions.
- Prepare and dispense medications, IV admixtures, and chemotherapy as required.
- Monitor and adjust patient medication regimens in collaboration with physicians.
- Participate in clinical ward rounds to provide pharmacological inputs.
- Ensure timely delivery of medications to wards, ICUs, and emergency units.
- Maintain and update patient medication records in the system.
- Support antimicrobial stewardship, pain management, and other clinical programs.
**Outpatient Services**
- Dispense prescribed and over-the-counter medications accurately.
- Provide patient counselling on dosage, side effects, storage, and adherence.
- Educate patients on preventive healthcare, vaccinations, and lifestyle modifications.
- Address and resolve patient medication-related queries and concerns.
**General Responsibilities**
- Ensure compliance with hospital policies, NABH/JCI standards, and legal regulations.
- Maintain adequate stock levels, including emergency and life-saving drugs.
- Monitor and minimize drug expiries, pilferage, and wastage.
- Train and supervise junior pharmacists, interns, and pharmacy assistants.
- Participate in pharmacovigilance activities and report Adverse Drug Reactions (ADRs).
- Support internal quality audits, accreditation processes, and continuous improvements.
**Qualifications & Skills**
- Bachelor’s or Master’s Degree in Pharmacy (**B.Pharm / M.Pharm / Pharm.D**).
- Valid registration with the **State/National Pharmacy Council**.
- Minimum **2 years of hospital pharmacy experience** (inpatient/outpatient preferred).
- Strong knowledge of **pharmacology, therapeutics, and drug administration**.
- Ability to interpret prescriptions and relevant laboratory reports.
- Excellent communication and interpersonal skills for coordination with healthcare teams.
- High attention to detail with a commitment to **patient safety and ethical standards**.

- Accuracy and error-free medication dispensing.
- Patient satisfaction and adherence to prescribed therapy.
- Timely drug availability in wards and outpatient departments.
- Compliance with audit and regulatory standards.
- Reduction in ADRs and medication errors.
- Efficiency in inventory management (mínimal stock-outs and expiries).
**Job Types**: Full-time, Permanent
Pay: ₹15,000.00 - ₹18,000.00 per month
