**Primary Responsibilities**:

- ED Communications and Relations
- ED Admission Monitoring
- TRC Coordination
- Hospital Admission Analysis
- Readmission Prevention
- After Hours Clinical Triage

**Essential Functions**
- Receive calls from ED Staff as soon as a patient of pratice is admitted to their ER.
- Provide the ER with PCP records to allow providers and medical staff in the ER to better educate themselves on the patient’s medical history.
- Bilateral communication between practice and ER regarding plan of action
- Communicate with hospitalist team regarding patients admitted to ER
- Ensure practice wide compliance with CMS requirements for Transition of Care
- Hand off to respective hospital follow up teams to ensure appointment is scheduled within preferred time range from discharge.
- Review all admissions for relevant history and document.
- Call discharged in preset cadence following discharge during a 30-day interval from date of discharge.
- Responsible for tending to patient calls received outside of standard practice wide office hours
- Participates in the care of the patient, under the direct supervision of the physician, or certified nurse practitioner, or Physician Assistant, and in collaboration with the healthcare team.
- Must have studies required by the provider available and needs to make sure they are in the patient’s chart (lab results, imaging studies, hospital discharge summaries)
- Must know how to read results and be able to communicate them effectively to the provider.
- Have clinical knowledge of chronic conditions such as Diabetes, Hypertension, Hyperlipidemia, Atrial Fibrillation, COPD, etc.
- Functions as a professional role model and resource person providing guidance to co-workers. Complies with established chain of command for work-related problems. Provides direct patient care as needed including co-management with other health care professionals.
- Collects and documents patient information, clinical data, and other relevant information such as vital signs, height/weight, allergies, active medication list, chief complaint, pain score, current symptoms, and registry updates.
- Provides and reinforces appropriate age-specific patient education, health information materials, and community resources, as directed. Identifies patient’s need for additional information and refers to RN.
- Performs time-sensitive delegated tasks, such as addressing prescription refill requests, patient questions, and EHR messages.
- Works with the healthcare team to ensure smooth and efficient patient flow and clinical operation.
- Manages information (including HIPPA) in accordance with state and federal regulations.
- Performs other duties as assigned, within the scope of practice and demonstrated competency.
