**Key Responsibilities**

Medication Management : Conduct comprehensive medication reconciliations to ensure accuracy and safety in patient medication regimens. Review and verify medication orders, identifying potential discrepancies and resolving them effectively.
- Patient Counseling and handling: Provide thorough counseling to patients regarding their medications. Educate patients on the importance of adherence to prescribed therapies.
- Medication Therapy Management (MTM): Develop and implement individualized medication therapy plans. Monitor patient outcomes and adjust therapies as necessary to achieve optimal health results.
- Collaboration: Work closely with physicians, nurses, and other healthcare professionals to coordinate patient care, participate in multi-disciplinary rounds, and contribute to clinical decision-making.
- Dispensing Operations: Oversee day-to-day operations of the dispensing pharmacy, ensuring compliance with standards and best practices. Coordinate with pharmacy technicians and interns, providing guidance and training as needed.
- Clinical Support: Provide drug information and consultative support to healthcare staff regarding medication therapies. Participate in formulary management and contribute to the development of clinical protocols.
- Quality Assurance: Engage in quality assurance activities, including documenting medication-related activities and participating in continuous improvement initiatives to enhance pharmacy services.

**Qualifications**
- Education: Doctor of Pharmacy (PharmD)/Bachelor of Pharmacy (B. Pharm)/Master of Pharmacy (M. pharm)
- Experience: Minimum of one year of experience in a clinical pharmacy setting, preferably in a hospital, pharmacy or ambulatory care environment.

- Strong clinical knowledge of medications and their therapeutic uses.
- Proficient in medication reconciliation processes and techniques.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively in a team-oriented environment.
- Strong problem-solving and critical thinking abilities.
- Familiarity with privacy laws and electronic health records.

**Preferred Qualifications**
- Experience in patient education and counseling.
- Knowledge of pharmacokinetics, pharmacodynamics and pharmacotherapeutics.
