**Key Responsibilities**:

- Maintain and organize medical records of all inpatients and outpatients.
- Ensure completeness of records by checking for missing reports or signatures.
- Index, code, and file records using ICD-10 and other classification systems as applicable.
- Ensure proper documentation practices (PDP) and compliance with NABH documentation standards.
- Assist in regular audits of patient records for quality and compliance.
- Support the legal and administrative requirements related to medical records.
- Safeguard patient confidentiality and secure record storage.
- Assist clinicians and authorized staff in timely retrieval of records.
- Coordinate with IT department if records are maintained digitally.
- Participate in training and development activities related to MRD practices.

Pay: ₹14,524.69 - ₹40,163.60 per month
