**Job Summary**:
The Home Care RMO provides medical care to patients in their homes, ensuring continuous health monitoring, early intervention, and coordination with healthcare teams to support patient recovery and chronic condition management.
**Key Responsibilities**:
- Conduct home visits for patient assessment and treatment.
- Monitor vital signs, manage chronic diseases, and adjust medications as needed.
- Provide emergency medical support until advanced care is available.
- Coordinate with specialists, nurses, and family members.
- Maintain accurate medical records and documentation.
