Consultant Physician

Overview:

You will be responsible for providing comprehensive diabetes care to adult and adolescent patients with Type 1 & 2, gestational diabetes, pre-diabetes, and metabolic syndrome. You will collaborate with dietitians and psychologists to develop personalized treatment plans and drive measurable outcomes through structured tele-consults and follow-ups.

Key Responsibilities:

• Clinical Care (Tele-OPD):
  • Diagnose, treat, and manage diabetes, initiate or adjust oral agents and insulin, design step-up/step-down protocols, and prevent hypoglycemia.
  • Interpret lab results, SMBG logs, CGM reports, and device data, create personalized targets, and titrate medication plans accordingly.
• Care Pathways:
  • Co-create customized nutrition, activity, and behavior-change plans with our team of dietitians and psychologists.
  • Coordinate referrals to ophthalmology, nephrology, podiatry, and cardiology services when necessary.
• e-Prescription and Documentation:
  • Issue safe and compliant e-prescriptions and maintain accurate EMR notes, problem lists, medications, allergies, and follow-up plans.
• Follow-ups and Continuity:
  • Plan regular reviews, medication adherence checks, and complication screening reminders.
• Patient Education:
  • Provide simple, culturally sensitive counseling on nutrition, exercise, foot care, sick-day management, and device use.
• Quality and Safety:
  • Identify red flags and escalate them appropriately, adhering to our platform's standard operating procedures and telemedicine regulations.

A Typical Consult Involves:

1. Reviewing vital signs, medical history, medications, SMBG/CGM trends, and recent lab results.
2. Agreeing on short-term goals, such as fasting and postprandial blood glucose levels.
3. Optimizing therapy by adjusting medications and writing electronic prescriptions with monitoring plans.
4. Setting lifestyle actions in collaboration with dietitians and providing education handouts.
5. Booking follow-up appointments and scheduling lab tests or retinopathy/foot checks as needed.
