Your focus will be on improving patient outcomes, enhancing healthcare delivery, and supporting a holistic approach to wellness.

Responsibilities and Qualifications:
  • Health and Wellness Assessments: Conduct thorough health assessments of members to identify their individual health needs, conditions, and wellness goals.
  • Care Plan Management: Develop and manage individual care plans for members, outlining required medical services, preventive health checks, and other wellness initiatives.
  • Clinical Liaison: Serve as the clinical touchpoint for members, answering their questions about health services, treatment options, and wellness initiatives.
  • A degree in a clinical field such as BDS (Bachelor of Dental Surgery), MDS (Master of Dental Surgery), BHMS (Bachelor of Homeopathic Medicine and Surgery), BAMS (Bachelor of Ayurvedic Medicine and Surgery), or equivalent.
  • Prior clinical experience, particularly in a healthcare coordination or patient care role, is required.
  • Strong clinical knowledge, excellent communication skills, strong organizational skills, and proficiency with electronic medical records (EMR) are essential.
