Senior Clinical Documentation Improvement Specialists lead efforts to ensure accurate and complete medical record documentation. This supports appropriate coding, reimbursement, and quality reporting.

Key Responsibilities:

  • Conduct concurrent and retrospective reviews of inpatient and outpatient medical records to identify opportunities for documentation improvement.
  • Evaluate the accuracy and completeness of clinical documentation to ensure it reflects patient condition, treatment, and outcomes.
  • Identify discrepancies, inconsistencies, and missing information in the medical record.
  • Analyze documentation to determine principal diagnosis, comorbidities, and complications.

Clinical documentation requirements, coding guidelines, and regulatory standards guide this role. Senior CDI Specialists perform comprehensive medical record reviews, collaborate with physicians, provide education and training, and serve as a resource for the CDI team.
