Job Summary:
The Patient Care Coordinator plays a pivotal role in ensuring seamless communication between patients and healthcare providers. This position requires excellent organizational skills, strong commitment to patient care, and the ability to multitask while maintaining a professional demeanor.
Key Responsibilities:
- Serve as the primary point of contact for patients, addressing inquiries, concerns, and providing information regarding services and procedures.
- Schedule patient appointments, confirm attendance, and manage follow-ups to ensure continuity of care.
- Collaborate with healthcare providers to ensure timely and appropriate care.
- Collect and maintain accurate patient records, including personal information, medical history, and insurance details.
- Assist patients in understanding their treatment plans, medications, and any necessary follow-up care.
- Coordinate communication between multidisciplinary teams to facilitate comprehensive patient care.
- Provide emotional support and guidance to patients and their families during their healthcare journey.
- Ensure compliance with healthcare regulations, quality standards, and best practices.
- Participate in patient education initiatives and outreach programs to promote health literacy and wellness.
- Assist with administrative tasks such as billing inquiries, insurance verification, and processing referrals as needed.
