Transplant Coordinator

Key Responsibilities

Pre Transplant

a) Coordinate patient evaluations,testing and listing for transplantation.

b) Educate patients and families about the Transplant process

c) Manage patient data, medical records and communication with referring physicians

d) Collaborate with a multi disciplinary team to ensure comprehensive patient care.

Transplant Process:

a) Coordinate organ procurement, surgical scheduling and patient preparations.

b) Communicate with organ procurement (OPOs) and transplant teams.

c)Facilitate transportation and logistics for organ transportations.

Post Transplant :

a) Monitor Patients Progress, addressing concerns and complications.

b) Coordinate follow ups appointments, testing and medication management,

c) Educate patients and families on Post transplant care and lifestyle adjustments.

Additional Responsibilities:

a) Maintain accurate records, documentations, database and UNOS( United Network for Organ sharing) Compliance.

b)Participate in quality improvement initiatives and performance metrics.

c) Provide emotional support and counseling to patients and families.

d) Collaborate with social workers,financial coordinators and other healthcare professionals.

e) Stay current updates with transplant regulations, policies and advancements.

Qualification:

MSW- Healthcare Administration or related field.

Knowledge on UNOS Policies and regulations

Bilingual - English/Kannada/Hindi

Having Transplant certifications will be an added advantage.
