To monitor the care delivered and provide the necessary clinical leadership to facilitate better recovery 5. To provide Goal based care for patients resulting in better outcomes 6. To implement clinical care protocols and pathways for common home healthcare presentations 7. Report on number of visits and maintain the documentation of the visits Timings: Work in any of the Shift timings are as under: 8:00 AM to 4:00PM 9:00 AM to 5:00 PM Transpor t will be provided by the organisation for the patient visits.
