**Medical Record Executive (MRD Executive)**

**Suyash Hospital - Raipur**

1. File receiving and Coding, Indexing.

2. Birth and Death Update.

3. Birth Registration.

4. File Segregation and arrangement.

5. Completion of Injury Reports.

6. Weekly Govt. Reports to RMC (Death & Birth).

7. Patient Query responding face to face.

8. Indent of Form-1 & 2 at Director of Health Services.

9. Maintaining file movement record.

10. Monthly repost submission to CMO, NHM.

11. Coordinate for all Insurance/Reimbursement form completion.

12. To produce file demanded.

13. File receiving from all the departments to Master register as well as on the system on daily basis.

14. Segregation of Normal Discharge/ MLC/ New born and Death Files separately

15. File checking/ Audit
- Active & Passive

16. Daily tracking of MRD file retrieval

17. To update daily IP files receiving and pending status

18. Death Registration in govt portal.

19. Birth Registration in govt portal.

20. Weekly Birth & Death Report To RMC

21. MLC Discharge/DAMA intimation to PS

22. Post mortem Intimation to PS with photocopy of ICP 2 sets

23. Completion of Injury Reports and maintained the dispatched records.

24. Reporting of Monthly / Communicable disease Reports to CMO

25. (Infant Death, Dengue, Malaria, MLC Statistic, Vector Borne Disease, TB, HIMS)

26. To Update daily birth and death report

27. To provide Monthly Quality Indicator and RAW data to Quality Dept. by 5th of every month

28. To respond Patient query by Mail/ face to face

29. To produce file when demanded.

31. To provide ICP to patient party when required

32. Presentation for (MRD Committee Meeting)

33. To coordinate with doctors regarding completeness of Consents /Counter signature etc.

34. To maintain departmental Register as per NABH guidelines

**Job Types**: Full-time, Permanent

Pay: ₹120,000.00 - ₹168,000.00 per year
