**A.** **Direct patient care**
- Report to the Supervisoron duty.
- Be punctual for the duties and adhere to the hospitals policies.
- Take charge of the patient and patient’s records.
- Check and take over the instruments and equipment from the previous shift.
- On admission receive the patient and make her comfortable. See that the beds are ready prior to the patient’s entry to the Labour Room
- Room Orientation to be given to the patients.
- Inform duty doctor / consultant about patient admission.
- Dietary information to be given to the nutrition team during the day & F& B staff in the night.
- To Perform initial assessment of patient immediately after admission to the Labour room.
- Attend doctor’s rounds & follow the orders.
- Do the fetal assessment as per the standards and document.
- Pain (contraction) assessment to be done as per the policy.
- All patient's Nursing care plans have to be documented & should be modified as per the condition of patients.
- Telephonic orders can be taken only at the time of emergency & it must be confirmed by another staff nurse or duty doctor and documented in the verbal order form and get it signed by consultant within 24hrs.
- Provide Patient education on parental and child safety, Nutrition, immunization etc,
- Check oxygen and suction on daily basis for working condition in shift as per the hospital policy.
- Carry out consultant's orders for all high risk patients, check vital signs and FHS frequently and inform to duty doctors and document as per the policy.
- All staff should know protocol on Non Stress Test (NST) Procedure and partogram.
- Close observation and monitoring of the Pre and Post Labour patients to be done according to the policy.
- Patients who are posted for LSCS, perform the pre operative preparation as per the policy and transfer to OT in a safe manner.
- Ensure the ID band on baby while receiving the patient from OT and identify baby correctly.
- Ensure to follow the policy of new born baby's admission procedure after delivery / LSCS.
- All the staff should know about the protocol in cases there is a still birth / IUD cases.
- All information to be exchanged & documented in each staffing shift, between shift & during transfer between unit / departments.
**B-**Medication Management**:
- Indent the medication as per the written order.
- Check patient’s case file and give medication
- Right drug
- Right dose
- Right patient
- Right time
- Right route
- Right to refuse
- Right assessment
- Right Education
- Right Evaluation
- Right drug documentation
- Enter in the medication chart immediately after administering the drug and observe for any reaction.
- Be proactive to clarify doubts with senior nurses, supervisors, Nurse educator etc. if there is any clarification required about the doctors orders, procedures, medication orders etc.
- Ensure that adequate supply of material/drug, available for patient care
- Ensure to follow Blood / blood products transfusion policy.
- All procedures to be entered in activity card immediately after procedure.
**C. **Infection control Practices**
- Ensure the cleanliness of unit & patient premises at all times.
- Assist for personal hygiene of the patient.
- Strict asseptic techniques to be followed while assissting for Labour procedures.
- Infection control protocols to be carried out: like Proper Hand hygiene, wearing PPE appropriately,Use of hand sanitizer before & after touching the patients & follow proper Biomedical waste segragation standards.
- Attend HIC training sessions & update the knowledge.
- Mention date and time of insertion of tubes & catheters.
- Assess Patient's IV Line frequently to prevent extravasations/ infiltration etc.
