Staff Nurse Critical Care Units / ICU
**A. Direct patient care**
- Report to the Supervisor/In-chargeon duty.
- Be punctual for the duties.
- Complete special assignments like Inventory checking, dressing trolley maintenance, crash cart inventory etc.
- Check and take over the instruments and equipment.
- Attend doctor’s rounds & follow the orders.
- Follow specific ICU policies & guidelines.
- Get clean the equipment as per hospital policy.
- Meet the basic hygenic needs of all patients like bath, mouth care, Catheter care etc.
- Take patient allocation from incharge / supervisors as per the patient acuity level.
- Provide individualized care with comprehensive & holistic approach.
- Up on admission / Transfer in of patients ensure readiness of unit, receive the patient and make him / her comfortable.
- Initial assessment to be completed immediately after the admission/ transfer in.
- Nurses should know the protocol on admission / discharge criteria.
- Should follow strict protocols on narcotics/ high risk medications & monitoring.
- Strict adherence to follow patient restraint policy.
- Ensure the guidelines to take care of critically ill patients with ventilator support.
- Use appropriate communication techniques to relate with patients.
- Nurses should know the protocol & policy on non availability of beds.
- Pain assessment - reassessment to be done and carry out intervention as per Doctors order.
- Indent only the required quantity of medicine and disposables and see that the returns are made early. Do not wait till the patient is discharged/ Transfer out.
- Provide individualized comprehensive care with holistic approach and document the care given according to the documentation policy.
- All patient care plans has to be documented & should be modified as per the condition of patients.
- Strict compliance to Blood / blood products transfusion policy.
- All information to be exchanged & documented in each staffing shift, between shift & during transfer between departments.
- Dietary information to be given to the nutrition team during the day & F& B staff in the night.
- Inform consultant about the admission / Transfer in, Transfer out cases.
- Special monitoring of vulnerable & patient on restrains.
- Ensure to do the assessment, re-assessment in every shift for the patients for Pressure Ulcer, DVT and Fall risk.
- Change 2 hourly positions after attending pressure points of bed ridden patient.
- Adhere to sample collection policy & protocols strictly.
- Collect the lab reports on time and inform the concern duty doctors/ consultants.
- Complete the nurse’s documents immediately after each procedure and entry to be done in the activity chart.
- Ensure that end of life care / Death care to be given with respect & dignity.
- All staff should know about all emergency codes & policies.
- To co-ordinate and work for the smooth functioning of the unit.
**B. Medication Management**
- Indent the medication as per the written order.
- Check patient’s case file and give medication
- Follow 10 rights before medication administration

- Enter in the medication chart immediately after administering the drug and observe for any reaction.
- Check for chemotherapy patient's chemo-plan as written by the respective consultants
Check for consent before chemo cycle.
- Monitor vitals every hourly & follow MEWS ( Modified Early Warning Score ) guideline & if there is any deviation inform ICU residents /consultant immediately and carry out the orders.
**C. Infection control Practices**:
- Ensure the cleanliness of unit & patient premises at all times.
- Infection control protocols to be carried out: like Proper Hand hygiene,
wearing PPE appropriately, Use of hand sanitizer before & after touching the patients
& proper Biomedical waste segragation standards.
- Ensure Strict adherence to Sterile dress code policy In the Critical care areas.
- Attend HIC training sessions & update the knowledge.
- Inform supervisor and duty doctor to check the blood and enter in the respective progress notes.
- Assess Patient's IV Line frequently to prevent extravasations/ infiltration etc and ensure to mention date, time of insertion of lines and tubes.
