**Job Summary**: As a Patient Relationship Officer at 24/7 DCT, you will play a crucial role in enhancing patient satisfaction and facilitating effective communication between patients and healthcare providers. Your contributions will be vital in creating a supportive environment for patients and their families.
**Key Responsibilities**:
**Patient Engagement**:
Serve as the primary contact for patients and their families, ensuring a welcoming and supportive atmosphere. Proactively reach out to address questions, concerns, and feedback regarding their care.
**Communication**:
Facilitate clear communication between patients and healthcare providers, ensuring that treatment plans and follow-up care are well understood. Provide information about services, resources, and support systems available through 24/7 DCT.
**Issue Resolution**:
Promptly address and resolve patient concerns and complaints, escalating issues to appropriate team members when necessary. Track and follow up on unresolved issues to ensure timely resolution.
**Coordination of Care**:
Assist in coordinating appointments, referrals, and follow-up care to ensure timely and effective treatment for patients. Collaborate with interdisciplinary teams to enhance overall patient care and experience.
**Data Management**:
Maintain accurate and up-to-date records of patient interactions, feedback, and outcomes in compliance with HIPAA regulations. Analyze patient feedback and trends to identify areas for improvement in service delivery.
**Education and Support**:
Provide patients with educational materials and resources to help them understand their health conditions and treatment options. Support patients and families in navigating the healthcare system and accessing available resources.
**Quality Improvement**:
Participate in initiatives aimed at improving patient satisfaction and care delivery. Contribute to the development of policies and procedures that enhance the patient experience at 24/7 DCT.
**Confidentiality and Accuracy**:
Demonstrate a strong commitment to maintaining the accuracy and confidentiality of medical records. This involves meticulous data entry, a thorough understanding of HIPAA regulations, and the ability to handle sensitive information with discretion.
**Qualifications**:
- Bachelor’s degree in healthcare administration, nursing, social work, or a related field preferred.
- Previous experience in patient relations, healthcare support, or customer service roles.
- Strong communication and interpersonal skills.
- Empathy and compassion in dealing with patients and families.
- Ability to work independently and collaboratively in a fast-paced environment.
- Proficient in using electronic health records (EHR) and patient management systems.
- Flexibility to work the night shift, including weekends and holidays.
