**Responsibilities**:

- To receive the diet information in case of IP patients.
- To check the diet prescription for the patient given by the Consultant.
- To visit the patient, check the patient history, preferences and perform nutrition assessment.
- To prepare the diet plans.
- To prepare the diet summaries/ patient meal cards and sends to Food and Beverages Department for the timely service of all meals.
- To check the tray setting against each meal card randomly when trolley is loaded and check the temperature of service trolley.
- Revision of the diet plan according to the patient’s requirement / change of Consultant’s diet order / counseling the patient and give nutritional hints about therapeutic diet.
- Perform diet counseling at the time of discharge.
- Tasting of food before service to the patient.
- Go for IDTR rounds to patients within 48 hours of admission when directed by the Dietician Incharge.
- To follow the Process Improvement programs and carry on Quality Initiatives in the department.
- To perform all other jobs as may be assigned due to exigencies of work.
- Daily dialysis rounds for daycare patients (in all 3 shifts).
- Counselling dialysis daycare patients and checking biochemical records.
- Calculating dietary intake by taking 24 hour recall method, once in 60 days.

**QUALIFICATION : B.Sc / M.Sc / DIPLOMA IN NUTRITION & DIETETICS**

**Experience : 0-1 year of experience.**
