To send weekly reports to BMC and other regulatory authorities regarding the births and deaths, vaccinations and notifiable disease cases occurring in the hospital with department coordinator.
- To collect injury certificate from concerned doctors as per approval from Chief Executive Officer for Police Station.
- To collect and compile the data from various departments and send the monthly statistics from MRD to the CEO / Medical Director and department coordinator.
- To issue the copy of indoor case papers to the patients where demanded in writing by the patient / patient’s heir, statutory bodies or court order. A printing charge of Rs. 5/- (Rupees Five only) per page will be taken. insurance for TPA or doctor after prior approval of the treating consultant, Medical Director or Chief Executive Officer.
- To enter the demographic and clinical data related to the inpatient admissions on a daily basis and disseminate to the marketing team after taking approval from operations head.
- To provide Statistical Data as per requirement to the concerned department and approval from department coordinator.
- To provide IP files to Doctors, Nurses with approval from Medical Director/ Chief Executive Officer with proper documentation in BIN card.
- Scrutiny od the IP files for completion and maintain error log accordingly.
- Particulars to be checked - e.g., in case of DAMA - Signature; Death - Cause of Death and FORM 4; MLC etc.
- To follow the checklist and inform the Medical Director / Head Operations for the error / missing record.
- Any other responsibility deemed by Medical Director or department coordinator.

**Job Specifications**: Graduate preferably diploma in Medical Records with 6~ 8 years of experience

**Supervised by** : Manager - Healthcare

Pay: ₹20,000.00 - ₹35,000.00 per month
