Job Position: Patient Care Coordinator

Patient Care Coordinator is a trained professional who helps to manage a patient's care. They monitor and coordinate patients' treatment plans, educate them about their condition, connect them with health care providers, and report their progress. Patient Care Coordinator will reporting to Head - Operations and dotted line reporting to the State Coordinator

We are seeking a dedicated care coordinator to assist us with patients’ enrollment at Centre of Excellence for
Rare Diseases. You will be responsible for onboarding patients and determining their needs, developing care plans, coordinating patient-care services, educating them about their condition, identify Funding needs and working closely with the ORDI team and the COE team to evaluate interventions.

The successful applicant will display a compassionate nature, be knowledgeable about health care practices, and provide exceptional patient service.

**Responsibilities**:

- Assist the Medical Team with Clerical works like filling up of mandatory forms, collect - update and report necessary data.
- Consult and collaborate with other health care providers and specialists to set up patient appointments and treatment plans
- Consult with patients and family members to discuss their health problems with Treating Physicians.
- Educate patients about their condition, medication, and give them specific instructions as advised by the

Treating Doctors.
- Discuss and develop a care plan to address Patient's personal health care needs.
- Check-in on the patient regularly and evaluate and document their progress.
- Coordinate for regular Medical Camps and awareness activities.
- Assist the care team with developing and accessing health interventions.
- Attend ongoing training and courses to keep abreast of new developments in health care.
- Assist with securing funding for medical care as required.
- Treat patients with empathy and respect and conduct oneself in a professional manner.
- Comply with organizational guidelines and health care laws and regulations.

**Requirements**:

- A Graduate preferable Social Work, or a related field.
- At least 2 years' experience in Care Coordination.
- Good verbal and written communication skills.
- Computer skills and proficiency in Microsoft Office.
- Strong analytical thinking and the ability to handle multiple tasks concurrently.
- Compassion and empathy.
- Ability to travel.

Remuneration will be as per Industry Standards based on Experience and Academic Qualifications.

Jodhpur.

Care Industry through various initiatives to sensitize the public at large to acknowledge rare diseases.

**Salary**: From ₹20,000.00 per month
