**Role Definition (Job Description)**:

- Staff nurses are accountable for assessing, planning, implementing and evaluating a plan of care for a specific patient assignment.
- Staff Nurses will wish the patient and introduce themselves to the patient before starting the shift
- They will take complete hand over from their previous shift nurses while going from bed to bed.
- Staff nurses will meet all admissions within 10 minutes, taking vitals, get patient changed into hospital dress.
- All the orders of medication, samples must be carried out within 2 hrs of admission.
- Any pending tasks must be written in notes or conveyed in writing when handing over the patients
- The nurse taking over must ensure that all pending tasks of previous shift are completed during her/his shift(e.g. collection of reports.etc)
- The staff nurses will check the special assignment and ensure it is done with utmost sincerity
- They will read all the orders of the last 24 hours and check if all the orders of doctors are carried out
- They will physically check all IV lines, drains, Ryle’s tube and catheters or any other invasive device. They will give care as per set protocols
- They will pay special attention to all patients receiving blood, chemotherapy, TPN, post operative patients and will take vitals as as per doctors instructions
- Staff nurses will inform any variation in vital signs, lab values ASAP to the ward RMO or directly to the consultant if RMO is not available
- Staff nurses will check the diet of their patients and inform in case a wrong diet is sent
- In case a patient is transferred from their unit, they will give complete hand over along with all patient belongings, reports, films etc
- They will keep special watch on MLC patient. No films will be handed over to such patients on discharge. They will inform the supervisor immediately when any MLC patient is getting LAMA or discharge
- Staff nurses will call the supervisor, ward RMO if their patient is becoming sick and will announce Code Blue if no help is available
- Staff nurses will set priorities when organizing care for patients.
- They will maintain continuity through clear and concise (verbal and written) communication.
- They will document patient care in a manner that is clear, complete, and concise and in compliance with nursing documentation standards.
- If the ordered drug is not available in pharmacy he/she must follow the policy for non-availability of drugs. In case the drug is not available within an hour, they will inform the nursing supervisor
- All staff nurses will ensure that a proper check is made before giving any drug( name, UHID, dose, route, time etc)In case of any doubt regarding medication, they must check with senior nurse of her/his ward,/RMO/Nursing supervisors
- They will read and follow the policy before giving any blood transfusions. If any shivering, rigor, chills, fever, tachycardia, tacypnea is reported during transfusions it must be brought to the notice of RMO/Nursing supervisor and reaction form must be filled
- Two nurses will check before giving any IV fluids especially chemo and any high risk drug
- Nurses will ensure that I/O chart is maintained only after confirming what the patient has exactly taken. Urine must be measured and only then written on the IO chart. The nurses will inform the patient to pass urine in the pot so that it is measured. They will document in nurses notes if patient refuses to save his urine for measurement
- Staff nurses will always check 2 identifiers for every care including giving the discharge summary to their patients
- They will ensure that all medications, other belongings their old records are handed over at the time of discharge
- They will give health education related to their illness at the time of discharge.
- They will participate actively in the unit-based in-service education and will attend all other in-service classes.
- For any vulnerable patient nurses will accompany self or will ask student nurses/GDAs to accompany the patient to the toilet to avoid patient falls
- Nurses will ensure that their dressing trolley is ready if accompanying doctors for dressings etc.
- They will always ensure that waste segregation and disposal of sharps is done appropriately.
- No nurse will ask GDAs to discard the waste after doing any procedure especially if it contains sharps
- Nurses will use equipment safely and efficiently. In case of any breakage/loss they will soon inform the unit/shift Incharge
- They will ensure that they speak professionally and will not make noise at the counter
- Nurses will not put their head down during night shift. They know that by doing this they may lose their job
- Nurses will take breaks by taking turns and will never leave the nursing station unmanned. In case they need to go to a room, they must ensure someone is there at the counter
- They will not eat /drink at the counters during any time of their shift
