**Overview**:
A Clinical Pharmacist plays a vital role in healthcare settings, working directly with patients and healthcare providers to optimize medication therapy and ensure the safe and effective use of medications. They collaborate with healthcare teams to provide comprehensive pharmaceutical care, focusing on improving patient outcomes and promoting medication safety.

**Key Responsibilities**:

- Medication Therapy Management:

- Conduct comprehensive medication reviews to assess the appropriateness, safety, and efficacy of prescribed medications.
- Develop individualized medication therapy plans in collaboration with healthcare providers to achieve therapeutic goals.
- Monitor patients' medication regimens and clinical progress, making adjustments as needed to optimize therapy and minimize adverse effects.
- Patient Counseling and Education:

- Provide medication counseling to patients, caregivers, and healthcare professionals, explaining medication indications, dosages, administration techniques, and potential side effects.
- Educate patients on proper medication adherence, lifestyle modifications, and self-management strategies to enhance treatment outcomes and prevent medication-related problems.
