- Looking for a registered nurse with 3+ years of in-ward experience as mentioned below:
**Main Responsibilities**:

- Assembling and organizing all medical documents and information needed to manage the case.
- Coordinating all parties involved in the medical case: the client and his family; the Case Manager; the medical specialists globally; and other medical parties. Schedule meetings and phone calls, confirming their execution.
- Overseeing the execution and outcome of the clinical plan as developed by the multidisciplinary team.
- Continuously update the Head Nurse and Operations Supervisor regarding the status of the medical cases, as well as reporting any problem or special situation in need of immediate attention to the case manager and or the Global Medical Director.
- Establishing a relationship with the client and his family and be fully acquainted with his/her medical case. Be a positive driver in a successful participation and working according to the highest service level agreements.
- Assisting the Case Managers in the management of the cases and the Global Medical Director in the quality control process.
- Proactively identifying and monitoring the client and his/her medical condition and well-being:

- Change in condition
- Deterioration or improvement
- Side effects of conditions or treatments
- Adverse events
- Change in general well-being e.g. eating habits, stool habits, sweating, sleeping habits, physical activity, etc
- Adverse events (e.g. Polypharmacy - taking several drugs at the same time etc.)
- Adherence and compliance to treatment
- Emotional changes and needs
- Monitoring of patient & family satisfaction
- Emotionally supporting the patient and his/her family as they cope with the challenges presented by dealing with a significant medical journey.
- Documenting all interactions and creating the necessary clinical reports in a computerized case management system as well as a CRM system. Confirming that the client receives and understands the medical summary.
- Assisting with the audit of client satisfaction surveys and prepare for submission to Care Quality Commission. Ensuring that all company policies and protocols are followed correctly.
- Explaining the exact nature and added values of our service, while assessing the needs of the patient, understanding and managing the expectations of the patient and / or his family.
- Making sure collaborate policyholder's participation, confirm timetable reliability and level of service as specified in the company's service agreement.
- Ensuring that all company policies and protocols are followed correctly.
- Pay special attention to information security at any moment anywhere you are, on-site or off-site as per all company security policies.

**Expectations**:

- To preserve a warm, empathetic and supportive relationship with the client and their family, providing availability at all times.
- To respect the client's rights, dignity and privacy at all times and maintain patient confidentiality.
- Thoroughly understanding the symptoms, the main and secondary diagnosis/conditions of the patient. Take elaborate and thorough Intakes and further medical updates from the patient and his family. Where needed looking it up on-line and further discussing with the case manager to better understand all medical aspects and plan next steps.
- Being able to assess whether you are dealing with any kind of medical emergency that warrants an immediate referral and refer a patient to the A&E.
- Being an integral part of the process of obtaining remote and face to face consultations. Taking active part in the decision-making process and medical recommendations. Nurses need to be able and feel comfortable to challenge the doctor’s (consultants') opinion and recommendation.
- Nurse must be accountable and fully responsible for every case unrelated to the doctors' role, accountability and responsibility.
- Maximizing the health of client by increasing healthcare education and promoting wellness.
- Oversees the satisfaction of client and family with the service
- The NURSE must take into consideration the diversity of the people she will communicate with on a daily basis (e.g., cultural differences of patients, consultants etc.)
- Improving and facilitating interdisciplinary communication and medical planning.
- The NURSE is the advocate, the eyes and ears of the patient, his caregivers, and family, making her a valuable resource for the medical team
