 OPD & IPD Patient file maintenance & documentation.

 ICD Coding with panicle and record maintenance in computer.

 Medical Records completion and to check the deficiency in the records.

 Filing and numbering of case sheet, assembling of case sheet.

 Birth and death Registration online by Civil Registration System portal.

 Medical legal record and its maintenance.

 Maintaining quality and accurate records by following hospital procedures.

 Hospital statistics, its analysis collection, and Interpretation of hospital statistics.

 Health Management Information System (HMIS) Reports prepare.

 To provide IPD / OPD patient data to the consultant / Resident / interns / department permission from authorization signatory.

 To handle & counseled the patient’s attendant problems regarding some issues like department related issue and reports related issue

**Experience**:

- Medical records: 3 years (preferred)
