Key Responsibilities include:
- Assess patients for transplant eligibility.
- Coordinate listing on organ donation registries.
- Organize medical tests, specialist consultations, and education sessions.
- Ensure timely completion of evaluations and documentation.
- Liaise with organ procurement organizations.
- Facilitate logistics for organ retrieval and transplant scheduling.
- Monitor patient recovery and health status.
- Schedule follow-up appointments and adjust care plans as needed.
- Work closely with nephrologists, surgeons, nurses, and social workers.
- Participate in team meetings to discuss patient progress.
