Health Coach – Nutrition & Preventive Care

Location:
Chennai (Hybrid: In-clinic + Virtual)

Role Overview

You will be the
first point of contact
for every patient entering the Prvnt ecosystem.

Your primary focus:
nutrition-led coaching
, supported by data from labs, wearables, and clinical insights. You'll onboard patients, translate medical advice into daily actions, run structured coaching sessions, and ensure every member feels guided and supported throughout their journey.

This is a
mid-level role
for someone with hands-on experience in nutrition and lifestyle coaching, who is confident managing patients independently while working closely with physicians.

Key Responsibilities

Patient Intake & Relationship Management

  • Act as the primary touchpoint for new and existing patients.
  • Conduct onboarding assessments, lifestyle and dietary history, and goal-setting sessions.
  • Build strong, trusting relationships and maintain high-touch communication.

Nutrition-Focused Coaching

  • Design and deliver personalized nutrition plans aligned with physician protocols.
  • Coach patients on metabolic health, weight management, gut health, hormone balance, and long-term preventive strategies.
  • Provide ongoing follow-up, accountability, and behavior-change support.

Monitoring & Insights

  • Review data from wearables, CGMs, food logs, and progress trackers.
  • Flag risks, trends, and opportunities to the clinical team.
  • Help patients understand their numbers and how daily choices affect them.

Program Execution & Follow-up

  • Conduct 1:1 virtual and in-clinic coaching sessions.
  • Track adherence, outcomes, and patient satisfaction.
  • Maintain Prvnt's "white-glove" experience across all touchpoints.

Operational & Clinical Support

  • Update EMRs and maintain accurate documentation.
  • Coordinate with physicians, labs, and operations for smooth workflows.
  • Support implementation of new protocols, programs, and experiments.

Qualifications

  • Education (must-have):
  • Bachelor's or Master's degree in
    Nutrition, Dietetics, Clinical Nutrition, Food & Nutrition, Sports Nutrition
    or related field.
  • Licenses/Certifications (preferred):
  • Registered Dietitian / Licensed Nutritionist (where applicable), or
  • Reputable certification in nutrition or health coaching from a recognized institution.
  • Experience (mid-level):
  • 3–6 years
    of experience in a clinic, hospital, wellness center, or health-tech setup.
  • Proven track record in
    nutrition counseling / health coaching
    with individual clients or patients.
  • Skills & Traits:
  • Strong knowledge of metabolic health, lifestyle medicine, and preventive care.
  • Excellent communication, empathy, and patient-handling skills.
  • Comfortable working with digital tools, apps, and wearable data.
  • Highly organized, proactive, and aligned with a high-performance, patient-obsessed culture.
