JOB PROFILE — Community Health Worker (CHW)Department: Outreach & Community ServicesReporting To: Outreach Coordinator / Program ManagerLocation: Clinic + Field (community, schools, slums, health camps)**1. Role SummaryThe Community Health Worker (CHW) acts as the connection between the foundation's healthcare services and the community. The role includes outreach, basic screening, patient mobilization, camp support, follow-up, and health education.2. Key ResponsibilitiesA. Community OutreachDoor-to-door community visits.Identify patients needing GP, Dermatology, Dental, Eye care.Mobilize people for clinics & health camps.Conduct women's group meetings & school awareness.B. Screening & TriageMeasure vitals: BP, sugar, height, weight, BMI.Identify red-flag cases.Register patients digitally or on paper forms.C. Health Camp SupportAssist in camp setup, registration & patient flow.Guide patients to correct specialty (GP/Derm/Dental/Eye).Assist doctors & nurses during high footfall camps.D. Follow-up & ReferralTrack chronic patients: diabetes, hypertension, skin ailments.Support referrals to partner hospitals/labs.Ensure follow-up visits to clinic.E. Health EducationConduct awareness on hygiene, skin care, dental hygiene, NCDs.Distribute IEC materials.Promote digital campaigns in community WhatsApp groups.F. ReportingMaintain daily activity log.Submit weekly & monthly progress reports.Maintain accurate outreach, screening & follow-up data.3. QUALIFICATIONS (Added)Minimum Education Mandatory:10th/12th PassBasic communication skills in Hindi & local language Preferred Educational Background:ANM (Auxiliary Nurse Midwife)GNM (General Nursing & Midwifery)BSW (Bachelor in Social Work)MSW (Master in Social Work)Certificate course in Community HealthDMLT or paramedical diploma (optional but beneficial)ExperienceMinimum 0–2 years in community outreach, NGO work, fieldwork, or healthcare.Experience in conducting camps, door-to-door awareness, or patient mobilization preferred.Technical SkillsAbility to measure BP, sugar & basic vitals (training will be provided).Basic smartphone use: WhatsApp, forms, photos, GPS.Ability to maintain registers & patient data.Soft SkillsGood communication & interpersonal skillsTrust-building ability in communitiesPatient empathyProblem-solving attitudeTeamwork & disciplineAdditional RequirementsComfortable working in field & travelling short distances.Should be physically fit to handle camps & outreach activities.Must have ethical behaviour & commitment to community welfare.SOP — Community Health Worker (Includes Qualifications)1. Patient Follow-Up SOPMonitor chronic disease patients.Ensure medication adherence.Schedule clinic visits.Maintain follow-up sheet.7. Reporting SOPDaily: outreach, screenings, follow-ups, referrals.Weekly: progress report to Outreach Coordinator.Monthly: detailed review and analytics.8.
